Each time a patient is admitted
to Drew Memorial Hospital for outpatient testing, he must go through
the registration process and be assigned an account number. Patient
registration is located at the front of the hospital in Admissions and
patients can be registered for outpatient testing at the Emergency Room
desk.
When a patient approaches
the registration area, he should have any paperwork given to him by
his physician ready to show the registration clerk. It is necessary
to have the signed order from your physician before any outpatient procedure
can be performed.
This is where he will need
to have his insurance or billing information ready. The following items
must be entered into the computer:
- Full name and address
(no nicknames should be entered)
- Copy of Social Security
card
- Copy of Driver's License
Card
- Employer
- Insurance cards (A MUST!)
- If liability or worker's
compensation, patient must provide the name and address of where
to send the bill.
- Name of an emergency
contact
- Name and address of
policy holder on the insurance card if different than patient
